Job Description
The Clinical Pharmacist role is essential to the patient care. The Clinical Pharmacist is valued for applying knowledge and skills to deliver appropriate, safe, efficient, effective and cost effective pharmaceutical care in the scope of their particular assignment. The Clinical Pharmacist is instrumental in the care of patients, as well as the development of effective, professional relationships within the multidisciplinary team. Acting as a resource to other staff members, the Clinical Pharmacist is an esteemed and valued clinical provider of patient care.
A clinical pharmacist position in cardiology is available at Intermountain Medical Center. The clinical pharmacist will provide inpatient pharmaceutical care services to heart transplant, heart failure, and mechanical circulatory support patients to promote the safe and efficacious use of drug therapy in the area of cardiology. Responsibilities include direct patient care, medication therapy consultations, protocol development, medication reconciliation, discharge counseling, and code participation. Serve as a member of the cardiology team by participating in group meetings, staff development programs, projects, service coverage, and journal clubs. Participation in departmental and interdisciplinary committees to support the organization's efforts for performance improvement to improve patient safety, therapeutic outcomes, and cost savings is required. Serve as a medication therapy resource for other clinical pharmacists, pharmacy students, pharmacy residents, pharmacy interns, physicians, medical residents, nurses, and patients. Opportunities for collaborative clinical research are available and encouraged. Precepting pharmacy residents and students on clinical rotations is required. Adjunct clinical faculty appointments are available.

Discuss mediation order clarifications with the prescriber, document any changes in the patient and pharmacy records, and inform others of medication order changes
Prevent, detect, monitor, document, and report adverse drug reactions and medication errors
Document a daily pharmacist progress note in the patients chart with recommendations
Counsel patients on discharge medications to facilitate discharge planning and a smooth transition to an alternate site of care
Actively support and participate in research opportunities with the heart transplant team

Minimum Requirements
Qualified applicants must possess a Bachelor's degree in Pharmacy or PharmD degree from an ACPE-accredited college of pharmacy.
Applicants must be eligible for pharmacy licensure in Utah and be able to dispense controlled substances.
Excellent interpersonal skills are required.
Works collaboratively with the other pharmacy staff and will report to the clinical coordinator of the cardiology team.
Position is Monday through Friday 8:00 AM to 4:30 PM with the requirement to work every fourth weekend
Preferred Qualifications
Completion of a Pharmacy Practice Residency with experience in cardiology and heart transplantation is preferred.
Completion of a specialty residency training program in cardiology and/or heart transplantation and/or critical care is preferred.
Pharmacy experience in cardiology preferred.
Pharmacy experience or training in a hospital environment preferred.
