Job Description
The Clinical Pharmacist role is essential to the patient care. The Clinical Pharmacist is valued for applying knowledge and skills to deliver appropriate, safe, efficient, effective and cost effective pharmaceutical care in the scope of their particular assignment. The Clinical Pharmacist is instrumental in the care of patients, as well as the development of effective, professional relationships within the multidisciplinary team. Acting as a resource to other staff members, the Clinical Pharmacist is an esteemed and valued clinical provider of patient care.
The Critical Care pharmacist will provide comprehensive pharmaceutical care to critically ill patients in the Shock Trauma ICU, Neuro Critical Care Unit, Respiratory ICU, Surgical Trauma and Surgical Transplant units. Responsibilities include direct patient care, attending multidisciplinary rounds, medication therapy consultations, protocol development, medication reconciliation, discharge counseling, responding to patient arrests as a member of the rapid response code team, and responding to traumas with the surgical trauma team in the emergency department. The Critical Care Pharmacist will serve as a member of the Critical Care team by participating in group meetings, staff development programs, projects, and journal clubs. Additional responsibilities include participation in departmental and interdisciplinary committees to improve patient safety, therapeutic outcomes, and cost savings. The clinical pharmacist is expected to serve as a medication therapy expert and to advise physicians, medical residents, nurses, and patients with regard to pharmacotherapy. Opportunities for collaborative clinical research are available and encouraged. The Critical Care Pharmacist will supervise pharmacy students, pharmacy interns, and pharmacy residents. Developing a clinical rotation to precept pharmacy students and residents is also required. Adjunct clinical faculty appointments are available.
Job Essentials
Provide comprehensive pharmaceutical care to critically ill patients in the shock trauma intensive care unit.
Provide direct patient care as part of the interdisciplinary emergency care team.
Collaborate with physicians, nurses, and other healthcare professionals to promote medication use in the Shock Trauma ICU that is evidence-based and aligned with national quality indicators.
Review medication orders for accuracy and appropriateness. Discuss medication order clarifications with the prescriber, document any changes in patient and pharmacy records, and inform others of medication order changes. Monitor drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions.
Provide consultation on patient-specific medication dosage, drug interactions, pharmacokinetics, and dosage adjustments as required.
Prevent, detect, monitor, document, and report adverse drug reactions and medications errors.
Participate in the drug therapy management of medical emergencies (codes, and traumas).
Maintain and update patient profiles with allergies, and current medications as necessary.
Work with physicians, nurses, and other healthcare professionals to develop and monitor medication-use systems that promote safe and effective medication us in the shock trauma intensive care unit.
Provide continuity of care for admitted patients as needed.
Counsel patients on discharge/transfer medications as necessary.
Provide operational support as determined by departmental need.
Expedite medication administration by all appropriate mechanisms (phone call, pneumatic tube, coordination with Central Pharmacy, hand delivery).
Resolve drug distribution related issues.
Participate in the selection, implementation, and monitoring of technology utilized in the medication-use process.
Promote and demonstrate rational, cost-effective drug therapy.
Identify and implement drug therapy where positive pharmacoeconomic impact could be achieved. The ultimate goal is to develop and assure cost effective clinical pathways.
Develop and conduct target drug programs and medication use evaluations as needed and report results to the appropriate Committees or Subcommittees.
Take a leadership role in the teaching programs of the Department of Pharmacy Services and Colleges of Pharmacy as they are related to critical care medicine.
Precept Doctor of Pharmacy clerkship students
Precept and mentor pharmacy practice residents
Educate patients, caregivers, and healthcare professional about safe and effective medication use.
Provide staff development support to pharmacists, nurses and physicians.
Facilitate the personnel development
Provide presentations, publications, and other informative activities related to critical care to intensive care unit staff.
Participate in unit based and hospital wide committees that impact medication use in the shock trauma intensive care unit.
Participate actively in critical care team meetings and conferences.
Conduct research on medication use and unit-based pharmacy activities that generate data on therapeutic, safety, humanistic, and economic outcomes.

Shift Details: This position will work a 7-on/7-off schedule. Shift hours are 12:30 PM - 11:00 PM
Minimum Requirements
Bachelor's Degree in Pharmacy or a Doctor of Pharmacy Degree from an ACPE-accredited College of Pharmacy.
Applicants must be eligible for pharmacy licensure in Utah and be able to dispense controlled substances
Current knowledge of state and federal regulations and statutes regarding the practice of pharmacy and the distribution of approved drugs, controlled substances, and investigational agents.
Maintains competency in unique aspects of drug therapy for critically ill patients.
Physical Requirements
Hearing/listening, manual dexterity, seeing.
Preferred Qualifications
Specialty Residency in Critical Care, Pharmacy Practice Residency, or 3 years experience in a critical care environment.
Board Certification is desirable.
BLS and ACLS certification desirable.
